The European Association of Holistic Music Therapy, z. s. offers its members three types of membership: PROFESSIONAL MEMBERSHIP HONORARY MEMBERSHIP

PROCEDURE FOR ACCEPTING MEMBERSHIP in the European Association of Holistic Music Therapy, z. s.

Applicant for membership:

1. Fill in the application form (downloadable below).
2. Send it with all attachments in electronic form to the e-mail of the professional committee: [email protected] The application can be sent electronically without signature.

The application must be accompanied by the following attachments:

• Recommendation of at least two members of the association - the applicant shall ask two existing members of the association to recommend his/her membership in writing (for basic membership in EACM, the recommendation of one member of the association and one professional member of the association is sufficient, for professional membership the applicant must provide the recommendation of two professional members of the association).
• Curriculum vitae with a focus on music therapy and practice, on courses and education in a supportive or related field, music therapy and holistic music therapy studies with PaedDr. Lubomír Holzer (courses, stays, lessons...), including education (high school, university, lifelong learning).
• Motivation letter - the applicant shall justify his/her interest in membership and interest in the field of music therapy (maximum one A4 size).
• Certificate of completion of studies CM 1, CM 2, CM 3 at the ICV UP Olomouc or other documents of completion of music therapy studies. If the applicant does not have such a certificate, he/she shall indicate whether and where he/she is currently studying music therapy.
• Evidence of experience in music therapy, helping professions, etc. The applicant shall indicate where he/she has practiced music therapy, how often, when and with which target group. If he/she has a certificate of experience - scan it or attach the original.
• Supervision - indicate how many hours, when and who provided supervision.

MEMBERSHIP FEE

The membership fee is 600,- CZK per year.

Dear members of the European Association of Holistic Music Therapy, z. s., we ask you to pay the membership fee for 2018 in the amount of 500,- CZK to the association's account 224320169/0600. Please use your mobile phone number as a variable symbol for payment and put your name in the note.
